UGA officials want to identify and support students, faculty, staff and visitors with disabilities who will need assistance during an emergency. Anyone with a disability, even those who have not otherwise self-identified or asked for an accommodation, are encouraged to complete an Emergency Assistance Referral Form. Kept on file by the Office of Emergency Preparedness, the completed forms only will be used to develop an emergency plan for those who want one. They will not be kept in student or personnel records.
